An NIHSS-8 ≥ 8 increases the suspicion of a proximal anterior large vessel occlusion (M1 or tandem carotid-M1) and may guide direct transfer to a thrombectomy center, but it does not replace a CTA, and a value < 8 does not rule out a large vessel occlusion.
